National Sun Yat-sen University Faculty and Staff Disaster Relief Support Subsidy Application Form
(For Disaster Relief in Guangfu Township, Hualien County only)
“On the Long Road to Recovery, NSYSU Walks beside You.”
To support faculty and staff members who wish to contribute their efforts to post-disaster recovery in Guangfu Township, Hualien, the Office of Sustainability offers transportation, insurance, and related subsidies to those who voluntarily participate in disaster relief as volunteers.
Faculty and staff are encouraged to utilize their professional skills and personal strengths to engage in diverse forms of support—such as home clean-up, medical assistance, tutoring for children, companionship for the elderly, or arts and cultural performances—to accompany disaster-affected residents as they rebuild their homes and recover from trauma.
【Subsidy Guidelines】
Faculty and staff of NSYSU are encouraged to engage in volunteer activities in the disaster-affected areas of Guangfu Township, Hualien County, starting from September 23, 2025 until the end of the year.
- Subsidy Period
September 23, 2025 – December 31, 2025
- Subsidy Items
(1) Transportation:
Subsidies cover round-trip transportation between Kaohsiung and Guangfu, based on actual travel receipts.
- By train: Tze-Chiang Limited Express fare Kaohsiung ↔ Guangfu: NT$759 × 2 = NT$1,518
- By car: Distance between Kaohsiung Station and Guangfu Station (280 km × 2 for round trip × NT$3/km) = NT$1,680
(Receipts or fuel invoices must be attached.)
(2) Insurance:
Applicants must purchase domestic travel insurance independently, with a coverage limit of up to NT$5 million, and a premium cap of NT$250 per person. The coverage period should be 3–5 days.
(3) Equipment and Materials:
To support various types of relief efforts (e.g., medical, educational, artistic, or community activities), necessary equipment or materials may be subsidized. Applicants must submit a list of items and intended uses in advance for evaluation and approval by the Office of Sustainability.
(Maximum subsidy: NT$1,000 per person.)
(4) Reimbursement Procedures:
- Complete this application form before departure.
- Be sure to purchase accident insurance before departure.
- After the activity, submit original receipts or invoices (train tickets or fuel receipts) to the Office of Sustainability (Social Practice Division).
- After verification, reimbursement will be transferred to the applicant’s postal bank account.
(5) Reimbursement Deadline:
All documents must be submitted by Friday, January 9, 2026.
Subsidy approval will be based on the actual verified expenses.

Each faculty or staff member may apply up to two times.
- Application and Reimbursement Process
Faculty and staff members intending to participate should first register using this form. After completing their service, they must submit:
- Train tickets or relevant receipts
- Activity-related documents and photos
to the Office of Sustainability for reimbursement and verification.
【Notes】
- To ensure proper use of subsidies, all receipts must correspond with the actual disaster relief dates (including up to three days before departure or the duration of ongoing relief). Receipts dated too far from the service period will not be eligible for reimbursement.
- As sufficient quantities of basic relief supplies (e.g., cleaning and disinfection equipment) are already available in the disaster area, such items are not included in this subsidy program. Priority will instead be given to teaching materials, tools, and other resources needed for post-disaster recovery and community rebuilding, to ensure fair resource allocation and avoid waste.
- Participants are advised to pay attention to personal safety and health during their volunteer activities.
- This program is intended for faculty and staff. Students who wish to apply for volunteer subsidies should contact the Office of Student Affairs for relevant guidelines.
